The term <strong>first gained formal recognition in 2014<\/strong>, when Dr. Jerrold T. Bushberg proposed <strong>ALADA<\/strong> at the National Council on Radiation Protection and Measurements (NCRP) annual meeting<a href=\"https:\/\/www.oooojournal.net\/article\/S2212-4403(14)00523-9\/fulltext#:~:text=26\">[1]<\/a>. Bushberg \u2013 a medical physicist and prominent member of the American Association of Physicists in Medicine (AAPM) \u2013 introduced ALADA as a variation of ALARA, emphasizing that <strong>radiation dose should be minimized while still achieving an image of adequate diagnostic quality<\/strong><a href=\"https:\/\/www.oooojournal.net\/article\/S2212-4403(14)00523-9\/fulltext#:~:text=26\">[1]<\/a>. This proposal was intended to stress <em>optimization<\/em> in medical imaging: rather than simply lowering dose endlessly, one should lower it <strong>only to the point that the image remains diagnostically acceptable<\/strong> (i.e. useful for diagnosis)<a href=\"https:\/\/www.iti.org\/iti-academy-modules\/narration\/D03-M05A.html#:~:text=appropriate%20equipment%20and%20exposure%20settings,Acceptable%20without%20compromising%20image%20quality\">[2]<\/a>. The concept quickly entered discussions in radiology and dental imaging circles, given growing concern that aggressive dose reduction under ALARA might sometimes compromise image quality and diagnostic efficacy<a href=\"https:\/\/isdent.org\/pdf\/10.5624\/isd.2015.45.4.263#:~:text=colleagues%20about%20the%20difference%20in,concept%20of%20ALADA%20would%20require\">[3]<\/a>. In fact, a 2015 editorial in <em>Imaging Science in Dentistry<\/em> explicitly advocated \u201ctime to move from ALARA to ALADA\u201d for dental cone-beam CT (CBCT), highlighting that two decades after CBCT\u2019s introduction, it was important to balance dose reduction with sufficient image quality for diagnosis<a href=\"https:\/\/isdent.org\/pdf\/10.5624\/isd.2015.45.4.263#:~:text=colleagues%20about%20the%20difference%20in,concept%20of%20ALADA%20would%20require\">[3]<\/a><a href=\"https:\/\/isdent.org\/pdf\/10.5624\/isd.2015.45.4.263#:~:text=for%20specific%20diagnostic%20tasks%20with,move%20from%20ALARA%20to%20ALADA\">[4]<\/a>.<\/p>\n\n\n\n<p><strong>What does ALADA mean?<\/strong> In essence, ALADA carries <em>the same spirit as ALARA<\/em> \u2013 keeping patient radiation exposure as low as possible \u2013 <strong>but with an explicit caveat<\/strong>: <em>do not reduce the dose so far that the resulting image is nondiagnostic<\/em>. In other words, <strong>the lowest acceptable dose is one that still yields a clinically useful image<\/strong><a href=\"https:\/\/www.iti.org\/iti-academy-modules\/narration\/D03-M05A.html#:~:text=appropriate%20equipment%20and%20exposure%20settings,Acceptable%20without%20compromising%20image%20quality\">[2]<\/a>. This term arose to remind practitioners that <em>\u201cpretty\u201d<\/em> or ultra-clear images are not always necessary if they come at the cost of higher dose; instead, the goal is an image that is just good enough for diagnosis, and no better (hence minimum necessary dose)<a href=\"https:\/\/isdent.org\/pdf\/10.5624\/isd.2015.45.4.263#:~:text=against%20the%20expense%20and%20risk,appropriate%20FOV%2C%20mAs%2C%20and%20kVp\">[5]<\/a>. ALADA thus highlights <em>optimization<\/em> (the second tenet of radiation protection, after justification) in a more practical way: <em>\u201cthe lowest dose necessary to produce diagnostic-quality images\u201d<\/em><a href=\"https:\/\/radiologykey.com\/radiation-protection-10\/#:~:text=b,as%20low%20as%20diagnostically%20acceptable\">[6]<\/a>. Notably, radiologists had long implicitly understood this \u2014 for example, standard definitions of ALARA in diagnostic radiology already imply using the <strong>lowest dose that still achieves the required image quality<\/strong><a href=\"https:\/\/radiologykey.com\/radiation-protection-10\/#:~:text=b,as%20low%20as%20diagnostically%20acceptable\">[6]<\/a>. ALADA simply puts this nuance front and center in the terminology. It has sometimes been phrased as <strong>\u201cALARA, but not beyond the point of diagnostic adequacy.\u201d<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><a>Early Appearances in Dental Radiology<\/a><\/h2>\n\n\n\n<p>The dental radiology community was among the first to embrace and formalize <strong>ALADA<\/strong>. With the expansion of dental <strong>cone-beam CT (CBCT)<\/strong> in the 2000s and 2010s, concerns grew about unnecessary radiation in dentistry, especially for children. Dental radiologists and organizations began using ALADA to guide practitioners toward <strong>appropriate, indication-based imaging protocols<\/strong>. For instance, the \u201cImage Gently in Dentistry\u201d campaign (launched in late 2014 as part of the Alliance for Radiation Safety in Pediatric Imaging) explicitly built its recommendations around keeping exposures <em>\u201cas low as diagnostically acceptable\u201d<\/em><a href=\"https:\/\/www.oooojournal.net\/article\/S2212-4403(14)00523-9\/fulltext#:~:text=imaging%20in%20dental%20practice,with%20patients%20on%20dental%20practice\">[7]<\/a><a href=\"https:\/\/www.oooojournal.net\/article\/S2212-4403(14)00523-9\/fulltext#:~:text=techniques%20are%20unable%20to%20answer,30\">[8]<\/a>. The campaign\u2019s six-step plan for pediatric dental X-ray safety emphasized that radiographs must be justified and then performed with the <strong>minimum dose that still yields a diagnostic image<\/strong>, in line with ALADA<a href=\"https:\/\/www.oooojournal.net\/article\/S2212-4403(14)00523-9\/fulltext#:~:text=imaging%20in%20dental%20practice,with%20patients%20on%20dental%20practice\">[7]<\/a>. Likewise, the FDI World Dental Federation\u2019s policy statement on radiation safety instructs that <em>\u201coptimization is the concept that a radiograph should be of sufficient diagnostic quality, keeping the dose to the patient as low as diagnostically acceptable (ALADA)\u201d<\/em><a href=\"https:\/\/www.fdiworlddental.org\/radiation-safety-dentistry#:~:text=Optimization%20of%20radiographic%20exposures\">[9]<\/a>. In practical terms, this means dentists are urged to use techniques such as smaller fields-of-view, pulsed exposures, faster image receptors, and patient-specific settings to <strong>reduce dose without sacrificing the clarity needed to make a diagnosis<\/strong><a href=\"https:\/\/www.fdiworlddental.org\/radiation-safety-dentistry#:~:text=Optimization%20of%20radiographic%20exposures\">[9]<\/a><a href=\"https:\/\/www.oooojournal.net\/article\/S2212-4403(14)00523-9\/fulltext#:~:text=,image%20receptor%20possible\">[10]<\/a>. The American Academy of Oral and Maxillofacial Radiology (AAOMR) and the American Dental Association (ADA) have also stressed similar principles in their guidelines \u2013 essentially an endorsement of ALADA under the umbrella of ALARA \u2013 when advising on the use of dental radiographs and CBCT. By 2015, dental imaging literature routinely mentioned ALADA; for example, Jaju <em>et al.<\/em> (2015) argue that with modern CBCT improvements, <em>\u201cit is time to move from ALARA to ALADA,\u201d<\/em> cautioning against obtaining unnecessarily high-quality (and high-dose) \u201cbeautiful\u201d images when lower-dose, diagnostically acceptable images will suffice<a href=\"https:\/\/isdent.org\/pdf\/10.5624\/isd.2015.45.4.263#:~:text=against%20the%20expense%20and%20risk,appropriate%20FOV%2C%20mAs%2C%20and%20kVp\">[5]<\/a><a href=\"https:\/\/isdent.org\/pdf\/10.5624\/isd.2015.45.4.263#:~:text=Implementing%20this%20concept%20of%20ALADA,move%20from%20ALARA%20to%20ALADA\">[11]<\/a>.<\/p>\n\n\n\n<p>It\u2019s worth noting that the ALADA concept even evolved further in the dental community. In 2017 a European pediatric dental imaging consortium (the DIMITRA project) proposed <strong>ALADAIP<\/strong>, meaning <em>\u201cas low as diagnostically acceptable, being indication-oriented and patient-specific.\u201d<\/em> This extension adds that exams should be tailored to the clinical <strong>indication<\/strong> and the <strong>patient\u2019s individual characteristics<\/strong>, reinforcing personalized optimization on top of the ALADA principle<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC11808696\/#:~:text=Principles%20of%20radiological%20protection%20and,oriented%20and%20Patient\">[12]<\/a>. The move from ALARA to ALADA \u2013 and then to ALADAIP for children \u2013 reflects the dental radiology community\u2019s commitment to minimizing dose while maintaining diagnostic benefit. In summary, dental experts have strongly endorsed ALADA in practice: all unnecessary exposure should be eliminated, <strong>but never at the expense of the diagnostic information<\/strong> that justified taking the radiograph in the first place<a href=\"https:\/\/www.fdiworlddental.org\/radiation-safety-dentistry#:~:text=Optimization%20of%20radiographic%20exposures\">[9]<\/a><a href=\"https:\/\/www.oooojournal.net\/article\/S2212-4403(14)00523-9\/fulltext#:~:text=imaging%20in%20dental%20practice,with%20patients%20on%20dental%20practice\">[7]<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><a>Radiology Community\u2019s Perspective<\/a><\/h2>\n\n\n\n<p>In the broader medical radiology field, <strong>ALARA remains a foundational principle<\/strong>, but ALADA has been recognized as a valuable clarification for diagnostic imaging. Radiologists and medical physicists agree that <strong>optimization<\/strong> of dose means achieving the <em>\u201clowest dose necessary for a diagnostically adequate study.\u201d<\/em> In fact, many radiology textbooks and training materials now mention ALADA alongside ALARA when discussing dose optimization. For example, a radiology safety text notes that <strong>keeping exposures ALARA inherently implies using the lowest dose that still produces a diagnostic-quality image<\/strong>, sometimes referred to as ALADA<a href=\"https:\/\/radiologykey.com\/radiation-protection-10\/#:~:text=b,as%20low%20as%20diagnostically%20acceptable\">[6]<\/a>. In practice, this perspective has always guided imaging protocols: radiologists want images that answer the clinical question, and <em>any additional image quality beyond that is a luxury that might not justify additional dose<\/em>. Thus, while the acronym ALADA may not be as ubiquitously cited in general radiology as ALARA, the <strong>underlying concept is very much accepted and taught<\/strong>. Radiology professional bodies like the <strong>American College of Radiology (ACR)<\/strong> continue to emphasize <strong>justification<\/strong> of imaging and <strong>optimization<\/strong> of dose. The ACR\u2019s Appropriateness Criteria and Imaging 2.0 initiatives, as well as the Image Wisely campaign (geared toward adult imaging), all align with <em>\u201cavoid unnecessary radiation, but do obtain whatever imaging is needed for proper diagnosis.\u201d<\/em> This is essentially an endorsement of ALADA in principle. Notably, the NCRP itself \u2013 which informs radiology practice \u2013 signaled in 2014 that a <strong>shift from ALARA to ALADA<\/strong> should be considered for medical imaging to underscore the importance of image quality in dose reduction strategies<a href=\"https:\/\/www.iti.org\/iti-academy-modules\/narration\/D03-M05A.html#:~:text=In%202014%2C%20the%20National%20Council,possible%20dose%20should%20be%20our\">[13]<\/a><a href=\"https:\/\/www.iti.org\/iti-academy-modules\/narration\/D03-M05A.html#:~:text=appropriate%20equipment%20and%20exposure%20settings,Acceptable%20without%20compromising%20image%20quality\">[2]<\/a>. In everyday radiology, one sees this balance in action: protocols for CT, fluoroscopy, etc., are tuned to use the lowest dose that still yields clear visualization of the anatomy\/pathology of interest. Radiologists caution against \u201cdose heroics\u201d \u2013 <strong>reducing dose to the point that images become nondiagnostic is counterproductive<\/strong> and could lead to missed diagnoses<a href=\"https:\/\/isdent.org\/pdf\/10.5624\/isd.2015.45.4.263#:~:text=against%20the%20expense%20and%20risk,appropriate%20FOV%2C%20mAs%2C%20and%20kVp\">[5]<\/a>. Thus, the radiology community\u2019s position is inherently aligned with ALADA: <strong>radiation should be minimized, but never below the threshold that compromises diagnostic confidence<\/strong><a href=\"https:\/\/www.iti.org\/iti-academy-modules\/narration\/D03-M05A.html#:~:text=appropriate%20equipment%20and%20exposure%20settings,Acceptable%20without%20compromising%20image%20quality\">[2]<\/a>. In summary, radiologists support ALADA as a useful articulation of the optimization principle, even if ALARA remains the more commonly referenced term in general. The key is that <em>image quality requirements set the floor for how low the dose can go<\/em>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><a>AAPM\u2019s Position and Involvement<\/a><\/h2>\n\n\n\n<p>The <strong>American Association of Physicists in Medicine (AAPM)<\/strong> has been deeply involved in promoting dose optimization concepts like ALADA. As mentioned, it was an AAPM leader (Dr. Bushberg) who initially coined ALADA in 2014 to address medical imaging practices<a href=\"https:\/\/www.oooojournal.net\/article\/S2212-4403(14)00523-9\/fulltext#:~:text=26\">[1]<\/a>. The <strong>AAPM fully supports the ALARA principle<\/strong>, and by extension it supports ALADA as a nuanced application of ALARA in diagnostic radiology. In fact, AAPM presentations and publications around that time echoed the ALADA message. For example, at the 2014 AAPM Annual Meeting, experts stressed that for CT scans, <em>\u201cthe medical benefit should be clear and dose [kept] ALADA (As Low As Diagnostically Acceptable)\u201d<\/em><a href=\"http:\/\/amos3.aapm.org\/abstracts\/pdf\/90-25494-339462-107960.pdf#:~:text=Summary%20%EF%81%B6%20Cumulative%20exposure%20to,not%20interpretable%20because%20of%20the\">[14]<\/a>. This statement encapsulates AAPM\u2019s view: patients should not be exposed to radiation unless it\u2019s justified by clear clinical benefit, and even then every scan should use the minimum dose that still achieves the needed diagnostic information<a href=\"http:\/\/amos3.aapm.org\/abstracts\/pdf\/90-25494-339462-107960.pdf#:~:text=Summary%20%EF%81%B6%20Cumulative%20exposure%20to,not%20interpretable%20because%20of%20the\">[14]<\/a>. The AAPM\u2019s involvement in initiatives like <strong>Image Gently (pediatric imaging)<\/strong> and <strong>Image Wisely (adult imaging)<\/strong> further demonstrates its commitment to these principles. Through these campaigns (in partnership with radiology organizations), AAPM has helped develop educational materials that explicitly mention keeping doses as low as diagnostically necessary. Moreover, the AAPM\u2019s own journal articles and task group reports on imaging optimization align with ALADA\u2019s philosophy, often implicitly. For instance, when discussing techniques like automatic exposure control, iterative image reconstruction, or pediatric protocol adjustments, the emphasis is always on <strong>finding the dose level that yields acceptable image noise and clarity for the task at hand, and not exceeding that<\/strong>.<\/p>\n\n\n\n<p>It\u2019s important to note that <strong>ALADA is not a replacement for ALARA in regulations or official policy<\/strong>, but rather a <em>complementary concept<\/em>. The AAPM and NCRP did not discard \u201creasonably achievable\u201d \u2013 instead, ALADA was proposed to <em>emphasize to practitioners what \u201creasonably achievable\u201d should mean in practice<\/em>. The AAPM continues to endorse ALARA as a general principle across all uses of radiation, while embracing ALADA in the context of diagnostic imaging quality control<a href=\"https:\/\/www.iti.org\/iti-academy-modules\/narration\/D03-M05A.html#:~:text=Minimizing%20Radiation%20Risk%2C%20Key%20Learning,that%20doses%20should%20be%20As\">[15]<\/a><a href=\"https:\/\/radiologykey.com\/radiation-protection-10\/#:~:text=b,as%20low%20as%20diagnostically%20acceptable\">[6]<\/a>. In practical terms, AAPM\u2019s position is that <strong>every imaging exam should be justified (benefit outweighs risk) and optimized<\/strong>. Optimization, as AAPM experts clarify, means you adjust scanning parameters and technologies so that <strong>the patient receives the lowest possible dose that still yields images of sufficient quality for diagnosis<\/strong><a href=\"https:\/\/www.iti.org\/iti-academy-modules\/narration\/D03-M05A.html#:~:text=Minimizing%20Radiation%20Risk%2C%20Key%20Learning,that%20doses%20should%20be%20As\">[15]<\/a>.
